We may use and share your information to provide wellness services, coordinate care, and support continuity of care with your permission.

  • Coordinating wellness services with a referring provider

  • Communicating with care managers or ACO partners (with authorization)

We may use and disclose your information to bill and collect payment for services, including:

  • Submitting claims to insurance companies

  • Determining eligibility or coverage

  • Collecting copayments or balances

 

3. For Healthcare Operations

We may use your information to operate our practice, improve services, and ensure quality, including:

  • Quality improvement activities

  • Administrative operations

  • Compliance and auditing

OUR DUTIES REGARDING YOUR INFORMATION

  • Use your information for marketing without your authorization

  • Share psychotherapy notes (if applicable) without authorization

 

BREACH NOTIFICATION

If a breach of unsecured PHI occurs, we will notify you as required by law.

 

SPECIAL NOTE ABOUT WELLNESS & HOLISTIC SERVICES

Inspired Now Wellness and Community Partners provide non-clinical wellness and holistic services. We do not diagnose, treat, or provide medical care. Information collected supports wellness services and coordination with healthcare providers when authorized.

We reserve the right to change this Notice at any time. Updated notices will be posted on our website and available upon request.
